style=\"height:20px;\" aria-hidden=\"true\" class=\"wp-block-spacer\"></div>\n\n\n\n<h2><strong>太多可能的相似病因</strong></h2>\n\n\n\n<p>有些人可能會覺得是不是高山症所引發不舒服，可能也會懷疑她是不是本來就有罹患心臟病、氣喘、甲狀腺機能亢進等…，或是也有人猜想她是不是上山之前喝了許多讓自己提神的飲料，像是咖啡、茶、蠻牛等。</p>\n\n\n\n<p>然而，進到急診室，並經過一連串的檢查與問診後，佳茗的確是一早先喝了一杯咖啡，但每天來一杯已經是她每天的習慣；除此之外，她也沒有我們所懷疑的心血管疾病等生理病史；至於高山症，赤柯山海拔約800至1200公尺，以高山症去解釋似乎又有些牽強，更何況佳茗可是幾乎長年居住在當地的原住民，突然不適應這樣的海拔也說不過去。</p>\n\n\n\n<p>排除可能引發的症狀後，佳茗向急診醫師描述的情況，多數沒有具體的生理原因，而且，與醫師會談的當下，佳茗並沒有再出現像剛才般極不舒適的症狀，即使在家醫科、心臟內科很可能也無法得到進一步的幫助，此種疑似因心因性所引發的呼吸急促、胸悶、顫抖等症狀，很可能是焦慮所引起的不適，這就是「身心科」或「精神科」的範疇。</p>\n\n\n\n<div style=\"height:20px;\" aria-hidden=\"true\" class=\"wp-block-spacer\"></div>\n\n\n\n<h2><strong>因生活壓力產生的「心」疾病－恐慌症</strong></h2>\n\n\n\n<p>提到身心科或精神科，你可能會想，那不是給有精神疾病的人看得嗎？其實不然，因為現代社會充斥了種種壓力，一旦面臨壓力，可能我們主觀上會感覺緊張、害怕，身體伴隨的反應很可能是心跳變快、盜汗，連帶著睡眠、飲食都會受到影響，腦中亦會想著要盡快讓自己拋開這件讓自己感到壓力的事情，反而越想腦中越是紊亂，如果已經對生活產生影響，此時，不一定是要到達精神疾病的程度，只要生活中已經頻繁的失眠、在特定工作場域中出現爆哭的情形等，也許，都可以考慮透過求助身心科或精神科，獲得進一步的幫助。</p>\n\n\n\n<p>近年來新聞媒體也不乏報導藝人罹患恐慌、恐懼症等焦慮症，輕則短暫休養、重則永久的淡出大螢光幕，雖然媒體報導的症狀、疾病等，只能呈現出部分的訊息，在未得到的完整的訊息或是本人親口現身說法前，我們無法依照現有的片面資訊妄下判斷，但另一方面，我們也能嘗試透過藝人的罹病的新聞報導，讓民眾開始去認識疾病。因此，這次就讓我們帶大家一窺近期討論度逐漸攀升的疾病——「恐慌症」的面貌。</p>\n\n\n\n<div style=\"height:20px;\" aria-hidden=\"true\" class=\"wp-block-spacer\"></div>\n\n\n\n<h2><strong>什麼是恐慌症？如何判斷恐慌症？</strong></h2>\n\n\n\n<p>根據精神疾病診斷與統計手冊第五版(DSM-5)，對於焦慮症中病症之一的「恐慌症(Panic Disorder)」定義為：反覆非預期性恐慌發作。恐慌發作是突然一股強烈的恐懼或強烈的不適，在幾分鐘之內達到高峰，並在這段時間內出現下列4個（或更多）的症狀：</p>\n\n\n\n<ul><li>心悸、心臟怦怦直跳、或心跳加快。</li><li>流汗。</li><li>發抖或顫慄。</li><li>呼吸短促或透不過氣的感覺。</li><li>哽塞感。</li><li>胸部疼痛或不適。</li><li>噁心或腹部不適。</li><li>感覺頭暈、步態不穩、頭昏、或快要暈倒。</li><li>冷顫或發熱的感覺。</li><li>感覺異常（麻木或刺痛感覺）。</li><li>失現實感或失自我感。&nbsp;</li><li>害怕失去控制或「快要瘋了」。&nbsp;</li><li>害怕即將死亡。</li></ul>\n\n\n\n<figure class=\"wp-block-spacer\"></div>\n\n\n\n<p>讓我們再次回到佳茗的狀況，雖然突然間身體出現極度不適，但進入醫院急診時，已經沒有明顯的症狀出現，生理檢查的結果也是正常，在急診科醫師的建議下，認為佳茗應該掛個身心科門診。<strong>恐慌發作後，入急診、做檢查時，有其困難，例如症狀在等待過程中已消失等，檢查結果大多顯示無異常</strong>，這也是恐慌症難以在醫療情境中被快速檢驗出來的原因。</p>\n\n\n\n<h2><strong>恐慌症是對於生理感覺反覆、持續地感到擔心</strong></h2>\n\n\n\n<p> 那次佳茗很快的就離開醫院，之後的每天仍是照常前往山上進行農務，但開車的過程中，對她來講變成了煎熬，因為佳茗仍擔心上次那種不舒服的感覺會再度襲來，因此當開始感覺自己出現些微的生理變化時（也許只是突然心跳加快了一下），有時她會停在路邊，等待生理反應回到本來的狀態，有時則再度直接下山掛急診。而這很有可能就是恐慌症，因為恐慌症的症狀之一，就是<strong>對於生理感覺反覆、持續地感到擔心</strong>。 </p>\n\n\n\n<p>佳茗本以為只有山路會引發自己的不舒服，因此對於上山途中的任何生理變化都是呈現草木皆兵的狀態，然而，在好不容易抵達目的地後，疲累的她先到農舍喝點水，但當拿起水杯的時候，佳茗感覺自己的手極度顫抖、心跳急遽加速，她想要發出聲音求救，但呼吸紊亂已經讓她話都講得不清楚了，農舍內的一切在她眼裡變得越來越模糊。這正是恐慌症的另外一個特點：<strong>發作時沒有預兆，什麼情境、地點都有可能發生</strong>！</p>\n\n\n\n<div style=\"height:20px;\" aria-hidden=\"true\" class=\"wp-block-spacer\"></div>\n\n\n\n<h2><strong>恐慌症發作怎麼辦？</strong></h2>\n\n\n\n<p>之後佳茗每想到這幾次經驗都讓她感覺害怕，但反覆急診都沒有檢查出特別的原因，於是她嘗試說服先生讓她醫院附近的農地工作，以避開可能發作的情境，且對她來說，如果真的發作了，她可以就近看診。這是比較偏向逃避的因應策略，短期間內雖然會讓個案感覺稍微安穩，但一發作起來又開始沒完沒了。因此事實上，比較好的策略是嘗試在發作的當下透過放鬆，或是其他等方式讓自己心緒平穩、度過。</p>\n\n\n\n<div style=\"height:20px;\" aria-hidden=\"true\" class=\"wp-block-spacer\"></div>\n\n\n\n<h2><strong>令人憂鬱的恐慌症</strong></h2>\n\n\n\n<p>前述恐慌症狀突然來襲、發作的無法預測性，與所帶來極度不適的生理感受，凡是經驗過的人都感覺到非常痛苦。其實恐慌症就像是敏感度很高的警報系統，而如果你的身體配備了一個這樣高免感度的系統，會發生什麼事情？</p>\n\n\n\n<p>那就是若生理出現任何隱微的變化，警報器就會啟動，並投入你「全身」的能量嘗試去抵禦、抑制。長期處在這種緊張的情況下，就像戰戰兢兢地在走一條看不見終點的平衡木上，一不留神就可能失足墜落，辛苦萬分。也因此，許多具有恐慌症的患者，不堪其症狀對生活造成的干擾，亦可能同時罹患憂鬱症。</p>\n\n\n\n<div style=\"height:20px;\" aria-hidden=\"true\" class=\"wp-block-spacer\"></div>\n\n\n\n<h2><strong>近看銀光幕中的恐慌症</strong></h2>\n\n\n\n<p>最後，搭上時事，大家可以翻開手邊關於近期TWICE團員罹病的報章雜誌與新聞，大至標題至小至內文，是不是感覺到哪部分充滿異樣？首先，團員的擔憂與害怕可能多是發生在登上舞台的那剎那，媒體報導多以舞台恐慌症稱之，但具情境特定性所引起的害怕，也許以畏懼症稱呼會更為適合；再者，媒體僅呈現恐慌症之字樣，並沒有具體提及該團員所具之症狀，加上經紀公司對藝人嚴厲的培訓，團員也很可能是是壓力過大而引發的身心反應。</p>\n\n\n\n<p>然而，單看媒體的報導，沒有透過親自的問診與互動，團員是否真的是罹患恐慌症，這點我目前仍語待保留，但值得鼓勵的是，透過喜愛的藝人了解疾病，也許會更了解、體貼她現階段淡出螢光幕調養身心的理由，對藝人來說，或許也是一種心理支持的方式。</p>\n\n\n\n<div
