style=\"height:20px;\" aria-hidden=\"true\" class=\"wp-block-spacer\"></div>\n\n\n\n<h2><strong>我向睡神招手，但沒有得到回應</strong></h2>\n\n\n\n<p>根據 2017台灣睡眠醫學學會調查，全台有超過10%人口受慢性失眠症所苦，也就是每9~10人中就有1位會遭遇睡不好的狀況，而且失眠比例隨著年齡增長而升高。調查結果也顯示<strong>女性</strong>的睡眠困擾高於男性，<strong>高齡者</strong>失眠比率明顯增加。女性睡眠困擾高於男性，可能和女性生理期、懷孕、更年期的生理及荷爾蒙變化有關，加上女性常是主要家務管理者，並身負帶養小孩的責任。高齡者可能正介於事業巔峰或屆退休年齡，還有生理功能退化的影響，壓力與疾病等因素皆是可能原因。</p>\n\n\n\n<p><strong>失眠症狀</strong>大致可以區分為四種類型，首先是<strong>入睡困難</strong>：不論白天或夜晚都擔心會失眠，這樣的朋友平常便容易焦慮緊張、坐立不安；決定「早點去睡覺，躺著休息也好」，結果常胡思亂想，愈躺愈緊張，怎樣也睡不著，焦慮症患者常有這類的睡眠障礙。二是<strong>難以維持睡眠</strong>：這類朋友通常睡眠很淺，有些微聲響或光線就會醒來，淺眠導致一整夜睡睡醒醒。三是<strong>睡眠品質不佳</strong>：會主觀地認為睡不好、不能熟睡、一直做夢，或是雖然有睡，睡醒起床後卻仍然覺得疲累，沒有精神。四是<strong>過早醒來</strong>：這類朋友常會半夜醒來後輾轉反側，難再度入睡，此現象常會在憂鬱症患者身上看到，必須留意，一開始可能單純以為自己是早起，想不到長久下來每天的睡眠時間都不夠。</p>\n\n\n\n<div class=\"wp-block-spacer\"></div>\n\n\n\n<p>從2015年調查中得知，有23.5%失眠者有使用助眠藥物的經驗，鎮靜安眠藥使用狀況從2016年8.53億顆，到2018年增加到9.18億顆，由此可推估國人平均每人每年服用39顆鎮靜安眠藥，且呈現增長趨勢。很多年前我在精神科病房和門診實習的時候，目睹民眾因各自的需求，會要求醫師如何開藥；現實生活中民眾自行購買或服用親友安眠藥的狀況也並不少見。安眠藥的需求普遍且大量，整個社會為此付出的成本不斐，甚至曾聽醫師說，如果出去自行開業，只要標榜失眠門診，業務量就夠了。</p>\n\n\n\n<p>實際上，64.5%的失眠者自己也對使用藥物助眠，有負面看法；不論是否曾服用過藥物，有8成以上的民眾擔心藥物成癮，以及藥物對肝臟、腎臟造成的負擔，也有超過5成民眾覺得使用藥物認知功能會受到影響，並導致隔天感到昏沉。就藥物治療上來看，是否能夠依照醫囑來服藥，以及使用時是否帶著擔心與懷疑的心態，肯定都會影響治療效果。</p>\n\n\n\n<div class=\"wp-block-spacer\"></div>\n\n\n\n<h2><strong>睡神的殿堂不在外面，在你裡面！</strong></h2>\n\n\n\n<p>我們的主觀認知，也就是<strong>如何解讀當下所發生的事情</strong>，往往<strong>決定了自己的情緒和行動</strong>。每個人造成失眠的原因可能都不同，若把睡眠障礙歸因於簡單、固定的特定原因，形成堅信不疑的觀念，反而可能讓自己對治療有著錯誤的認知、因為害怕而裹足不前，而加強症狀，使治療的困難度增加。</p>\n\n\n\n<p>舉例來說，很多人認為睡眠障礙都是因為外在壓力引起，而將壓力當成是最大、唯一的導火線，認為只要將外在壓力排除，或是不去觸碰，自然能夠不藥而癒。此種過於簡化的認知方式，有時反而造成治療上的延誤。事實上，睡眠障礙的成因並非如此單純，關係著飲食、習慣、身體變化、呼吸方式、關係品質和如何解讀當下所發生的事情等，彼此相互影響，具相當的複雜性。</p>\n\n\n\n<p>換句話說，<strong>失眠是一個現象，是一連串行為、情緒和想法交替作用下的結果</strong>。如果只針對最表層的症狀做處理，就像是哪裡失火，就往哪裡滅火，但底層仍不斷在悶燒，一旦各種條件具足的時候便又會冒出新的火苗－再次失眠。</p>\n\n\n\n<p>我們的主觀認知，也就是<strong>如何解讀當下所發生的事情</strong>，往往<strong>決定了自己的情緒和行動</strong>。再者，把睡眠障礙標籤化，即有許多自己堅信不疑的觀念，可能讓自己對治療有著錯誤的認知，因為害怕而裹足不前，反而加強症狀，使治療的困難度增加。</p>\n\n\n\n<div style=\"height:20px;\" aria-hidden=\"true\" class=\"wp-block-spacer\"></div>\n\n\n\n<h2><strong>你知道睡覺也有階段性嗎？</strong></h2>\n\n\n\n<p>你是否也曾經主觀認為「睡得好就是能夠睡得著」，「昨夜我都在做夢，都沒有睡」？我們先來了解整晚睡眠的週期變化：</p>\n\n\n\n<p>第一階段是入睡期︰閉上眼睛後讓自己逐漸進入平穩放鬆的狀態，介於半夢半醒之間。此時呼吸有規律，脈搏均勻、皮膚因流汗而顯濕潤、四肢肌肉放鬆、姿勢變化減少。</p>\n\n\n\n<p>第二階段是淺睡期︰眼球快速轉動、呼吸轉為斷續不規律、肢體肌肉雖然放鬆但偶爾會有手指、嘴角抽動現象，可能做片斷的夢，眼球會慢慢地由一邊轉向另一邊。</p>\n\n\n\n<p>第三階段是由淺睡期進入熟睡期︰呈現慢波睡眠，腦部仍然持續活動著，此時身體極度放鬆，體溫及血壓開始下降。此階段的睡眠進入深度睡眠，我們的感官對外界刺激的知覺降至最低，不容易被喚醒。</p>\n\n\n\n<p>第四階段是熟睡期︰身體極度放鬆，處於癱軟狀態，進入快速動眼期，此時大腦神經元活動程度和清醒時相當。若是這時候被喚醒的人，常會說剛剛正在做夢，有比較深刻的夢境記憶與感受，而尿床及夢遊都出現在此期。</p>\n\n\n\n<p>以上四個階段為一個週期，歷時約90~120分鐘，如此週而復始。整晚睡眠就在平穩與變動狀態中交替呈現4至5個週期。在睡眠的前半夜，有較多的慢波睡眠，讓身體休息放鬆；而半夜和清晨時則有較多的快速動眼期，來進行情緒的處理和記憶的去蕪存菁。隨著年齡的增長，每個人的睡眠周期也會發生變化。</p>\n\n\n\n<div style=\"height:20px;\" aria-hidden=\"true\" class=\"wp-block-spacer\"></div>\n\n\n\n<h2><strong>一些迷思會讓你與一夜好眠擦身而過</strong></h2>\n\n\n\n<p>「昨天晚上我都在做夢，都沒有睡」，這句話是非常主觀的認定，但其實做夢本身就是睡眠的一部份。另一方面，睡得著不見得會睡得好，心情是否安定，身體能否放鬆會直接對應到睡眠的品質。若從身體肌肉和腦神經放鬆的層面來談，現代人很多其實都沒有真正體驗到放鬆的感覺，若沒有親身經驗和長時間的練習，往往都是身體仍處肌肉緊繃而不自知，例如睡眠中牙關處於緊咬、有磨牙或打呼現象。</p>\n\n\n\n<p>此外，睡眠障礙也跟身體大有關係，舌根和上喉嚨都是脂肪，如果脂肪比例過高，肌肉就會受到擠壓，造成鼻道或喉部上呼吸道阻塞。當呼吸道持續一段時間都無法通暢，阻力過高就必須增加呼吸的驅動力來維持正常氣流量。嚴重者會導致睡眠時長期的缺氧，影響到白天的精神及衍生出慢性疾病，例如高血壓、心臟病，最嚴重的是中風。說到這裡，你是否也了解到，想要睡得好，解決失眠，其實方方面面都要照顧到，絕對不是單一因素就會造成長期失眠。</p>\n\n\n\n<div class=\"wp-block-spacer\"></div>\n\n\n\n<h2><strong>改善失眠先從起床開始</strong></h2>\n\n\n\n<p>一個人的身心靈健康與飲食、作息習慣、生理發展、呼吸方式、關係品質等，息息相關，任一方面過與不足，皆會導致整體的失衡。每天早晨鬧鐘響的時候，如果你需要按貪睡鈕、想繼續賴床，你可能就是睡眠不足。而你也不會因為再多休息幾分鐘，而變得更有活力，這樣是否了解了睡眠的重要性呢？</p>\n\n\n\n<p>如果想要一夜好眠，跟著日月運行的自然作息是最好的，但現代人還有輪班、夜班等狀況，不見得可以晚上睡覺；甚至對許多晚睡的現代人來說，上床睡覺的時間是一個巨大的障礙。那麼，不妨先從<strong>起床</strong>開始吧：維持規律且固定的時間起床，喚醒自己的方式可以先打開燈光（如果室內是暗的），在床上做些輕度的伸展運動，讓自己溫和地醒過來。然後逐步辨識出自己生活中，造成失眠的原因，這時慢慢更新自我的身心程式設定，適時的讓自己放鬆，許多時候過度執著只會適得其反，如此我們就能與一夜好眠越走越近。</p>\n\n\n\n<div
